I uploaded this on request.  I'm only missing the side quest were you escort the fisstech dealer in Novigrad on the docks all others are done. All alchemy crafted to the highest level, all Witcher gear crafted to the highest level, all Gwent cards collected, all locations discovered,and the Enchanter is complete. Story decisions: Ciri is Empress, Geralt romanced Yennefer, Cerys is Queen, Radovid is dead, Roche, Ves, and Thaler are alive,  Keria Metz is alive and with Lambert, the children were saved and the Baron and Anne are alive, Triss and the other mages fled to Kovir, and Sarah is with Corene in the haunted house, Olgierd is alive. Witcher 1 Neutral path, Witcher 2 Iorveth path, freed Saskia, Stennis died, Letho is alive.

Update: As it turns out I was missing the following quests: Last Rites, Finders Keepers, Novigrad Hospitality, and A Bard's Beloved. These have now been completed. I apologize for this.
