This Save Game is 100% legit. No cheats, exploits or savegame editors were used in the process.

As a longtime witcher fan, I tried to play in the most lore friendly way possible. This is ofcourse subject to debate, but the way I see it Geralt has to end up with Yennefer and every quest was completed in a way that feels natural to Geralt ( as presented in the books) and allowes for the least possible deaths of characters.

Things that were completed:

The base game and both expansions
99% of the sidequest found and successfuly completed
Every piece of witcher gear crafted and upgraded to Grandmaster Level
Corvo Bianco fully upgraded
A vast majority of Relic Tier items and unique items have been collected and sit in the player stash.
Nearly every armor, weapon and rune diagram collected
Runesmith fully upgraded
All Gwent Cards
No failed Quests
Most of the monster trophies
Most of the horse gear available for Roach

Endstates:

The base game ended with Ciri becoming a Witcher. None of the main cast died. (only counting avoidable deaths ofcourse). Radovid was killed. Geralt ended up with Yennefer.
Hearts of Stone ended with Olgierd free of his pact.
Blood and Wine ended with everyone alive and Regis having left the country.
