If you wish to play  Hearts of stone there is save file just after main story ends and you are in Kaer Morhen
If you wish to play Blood and wine there is save file for it just after Hearts of Stone is Finished (good ending)

Main Story Proceeds like this : You end up with Yen, blizzard is stopped and ciri becomes Witcher (good ending) Skelleg is ruled by queen so on so forth Radovids ded 
Also ive aquired a lot of gold for you (all the side missions are Complete (witcher contracts) included) all side stuff completed  in white orchard and velen 
there are still some left on skelleg but it shouldnt be too much of a hassle.
